The terms for using MyDocDay.

You must be at least 18 years old and legally able to enter into these terms to create or use a MyDocDay account.

If you enter information for a child, dependent, family member, or another person, you represent that you are authorized to organize or manage that information and to provide it to MyDocDay.

MyDocDay provides personal tools for staying on top of healthcare follow-through. These tools may organize appointments, providers, clinics, notes, locations, appointment reviews, health actions, medication schedules, reminders, and care responsibilities for family members you are authorized to support.

MyDocDay does not provide official medical records, secure provider communication, appointment confirmation, test results, prescriptions, billing services, or clinical instructions. Features may change as the service develops, and some features may be identified as early access, preview, or coming soon.

MyDocDay may use information you provide, such as dates, cadence, appointment details, medication timing, completed health actions, appointment-review responses, and family care responsibilities, to organize items, calculate upcoming dates, identify due or overdue items, group related family actions, and display prompts about when you may want to schedule a next step.

This guidance is automated organizational assistance based only on the information available to MyDocDay. It is not medical advice and does not assess symptoms, medical urgency, diagnoses, treatment, medication safety, or the clinically appropriate timing of care. You are responsible for confirming care and scheduling decisions with a qualified healthcare professional.

Creating, editing, rescheduling, cancelling, or deleting an appointment in MyDocDay changes only your organizational record. It does not book, confirm, cancel, or reschedule an appointment with a provider or clinic. You must communicate directly with the provider and confirm all appointment details.

Post-visit reviews record the outcome and next steps you enter. Any Health Action or replacement appointment created from a review is an organizational aid, not a provider instruction or medical record. Review prompts may be delayed, unavailable, or based on an appointment date that was entered incorrectly.

MyDocDay is not a healthcare provider and does not provide medical advice, diagnosis, treatment, prescribing, clinical monitoring, or emergency services. Information displayed by MyDocDay is for personal organization and should not replace advice or instructions from a qualified healthcare professional.

Do not use MyDocDay for an emergency or time-sensitive medical need. Contact emergency services or an appropriate healthcare professional directly.

You are responsible for providing accurate account information, protecting your password and signed-in devices, and notifying us if you believe your account has been accessed without permission. You are responsible for activity performed through your account unless prohibited by applicable law.

You may not impersonate another person, create an account through fraud, or use another person's account without authorization.

You retain ownership of information and images you enter or upload. You give MyDocDay permission to host, process, reproduce, and display that information only as needed to provide, secure, maintain, and improve the service and comply with law.

You are responsible for reviewing care information for accuracy. MyDocDay does not verify provider details, appointment details, locations, notes, health actions, reminders, or information entered for another person. Information displayed in MyDocDay may be incomplete, outdated, entered incorrectly, or different from the information maintained by a healthcare provider.

Health actions, reminders, notifications, calendar entries, and displayed dates are organizational aids. They do not provide clinical monitoring, and delivery or timing is not guaranteed. Device settings, connectivity, operating-system behavior, time zones, service interruptions, or information you enter may delay or prevent delivery. You remain responsible for confirming appointments, meeting time-sensitive care needs, and following instructions directly with your healthcare providers.

General notification wording is used by default. If you enable detailed notification messages, care information may appear on your lock screen and may be visible to anyone with physical access to your device. You are responsible for selecting notification and device privacy settings appropriate for your circumstances.

Medication names, strengths, forms, instructions, schedules, statuses, and reminder times in MyDocDay are user-entered personal organizational information. MyDocDay does not prescribe, dispense, verify, or recommend any medication, dose, frequency, route, timing, start or stop date, or instruction. It does not check allergies, contraindications, duplications, interactions, side effects, expiration, or whether a medication is appropriate or was taken.

Pausing or completing a medication in MyDocDay only changes the app record and related reminders. It is not advice to pause, stop, start, or change a medication. Do not rely on MyDocDay as your sole method for remembering medication. Always follow the label and instructions from your qualified healthcare professional or pharmacist and contact them directly about missed doses, adverse effects, or medication questions.

You may not:

• Use MyDocDay unlawfully or to harm another person.
• Enter information you are not authorized to possess or manage.
• Attempt to access another user's account or information.
• Interfere with the service, security controls, or infrastructure.
• Reverse engineer or misuse the service except where law permits.
• Upload malicious code or content that violates another person's rights.
